I consent to the use of any video /photography in which my child may appear by the Diocese of Trenton and / or parish. I understand these materials are being used for promotion of the parish Religious Education Program and / or parish activities, which may include recruitment and fundraising efforts.
summer intensive agreement
By registering my child for the Summer Intensive Program I agree that my child is available / able to participate in the all the days / times of the program. I understand the Summer Program requires my child to participate in six additional sessions / parish activities during the year.
